Insurance Requirements Per Project
Required is the actual endorsement to your General Liability insurance policy naming EK Bailey Construction and (PROJECT OWNER) as additional insured and endorsing your policy as primary and Noncontributory. Have your insurance company use an ISO Form B CG2037 07/04 Products Completed Operations or its equivalent and an ISO Form CG2010 07/04 Premises or its equivalent. Have your insurance company provide a Waiver of Subrogation ISO Form CG2404 10/93 or its equivalent and a Per Project Aggregate ISO Form CG2503 05/09 or its equivalent. These are MANDATORY requirements.
You also need to reference the individual project name on your certificate of insurance and provide a 30 day prior notice of changes or cancellation. The subcontractor shall, during the continuance of work under the contract, including extra work in connection therewith, maintain in companies acceptable to the Owner (Best rating of A or better) the following insurance coverage:

A. WORKERS COMPENSATION AND EMPLOYERS LIABILITY
- Workers Compensation – Statutory
- Employers Liability $1,000,000.00
B. COMPREHENSIVE GENERAL LIABILITY
- General Aggregate: $2,000,000.00
- Products: $2,000,000.00
- Personal Injury: $1,000,000.00
- Each Occurrence: $1,000,000.00
C. COMPREHENSIVE AUTOMOBILE LIABILITY
- Combined single limit: $1,000,000.00
